Case Study 1

A man named John Green, who is 33 years old and Caucasian, visits the office to become a new patient. Even though John was born female, he identifies as male. He transitioned from female to male two years ago and completed the transition with his family and socially last year. He recently moved back home and is currently unemployed. He has been getting testosterone from the internet to use himself. Except for getting suppression medications online three months ago, he hasn’t had any medical care since his transition. He has a history of smoking two packs of cigarettes daily for a decade, smoking 3-6 marijuana joints each weekend (legally), and dealing with episodes of depression. He’s been HIV-positive for three years, but his last blood test six months ago showed that his viral levels were low. Lately, he’s been feeling very weak, which led him to move back in with his parents. He takes Biktarvy daily (which he gets for free in the mail) and 100 mg of Testosterone IM every week. He doesn’t have any significant medical history, has never been married, and his family history doesn’t raise any concerns. John is worried that his return home and unemployment might burden his family and affect his health.

Preparing for the Case Study:

Think about your experiences as a nurse and what you’ve learned about diversity in health assessments.
Your instructor will give you a case study by the first day of this week. Check the “Course Announcements” section in your class to find out which case study you have.
Think about the patient’s background, lifestyle, culture, and other factors that could affect their health.
Imagine how you would gather information about the patient’s health. What questions would you ask, and how would you ask them in a way that’s respectful of their background and culture? Come up with five questions that you would ask the patient to understand their health history and potential health problems.
Consider the challenges of talking to patients from different groups. What strategies can you use as a nurse to understand their cultural factors while getting the right information?

By the Third Day of Week 2:

Share an explanation about the patient you were assigned, focusing on their background, lifestyle, culture, and other relevant factors. Explain the things you need to be careful about when talking to this patient and why. Provide at least five specific questions you would ask the patient to learn about their health history and possible health risks.
